Crocus Foundation’s Prescribed Exercise Program provides medically supported, individualized rehabilitation designed to help cancer survivors rebuild strength, improve function, and move forward with confidence during and after treatment.

Exercise Designed Around You

Our Prescribed Exercise Program is developed in partnership with oncology providers and led by certified Cancer Exercise Specialists to support individuals during and beyond cancer treatment.

Whole-Person Approach

Beyond exercise, we take a whole-person approach to recovery by incorporating guidance around nutrition, lifestyle habits, and emotional well-being to support long-term health.

Tailored Treatment

Each participant receives a personalized exercise plan tailored to their diagnosis, treatment history, physical condition, and goals—helping safely rebuild strength, endurance, balance, and mobility.

Begin with a Medical Referral

Participation in the Crocus Survivorship Program requires a completed medical referral from your oncologist or healthcare provider.

This referral helps our specialists understand your diagnosis, treatment history, and overall health so we can personalize your care safely and effectively, while keeping your medical team informed of your participation.

What to Expect at Your Discovery Visit

Once your referral is received, one of our Cancer Exercise Specialists will contact you to schedule your Discovery Visit.
